Overview

Falls are a significant safety concern across healthcare and caregiving settings, including hospitals, rehabilitation programs, long-term care, assisted living, home care, and community-based services. Fall risk is rarely caused by a single factor. Mobility limitations, medications, cognitive changes, acute illness, chronic conditions, continence needs, vision changes, environmental hazards, footwear, assistive devices, and changes in functional ability may all contribute.

Effective fall prevention requires a multidisciplinary approach. Nurses may identify clinical changes, medication effects, orthostatic symptoms, continence concerns, and changes in overall condition. Nursing assistants and direct caregivers may observe changes in gait, transfer ability, behavior, toileting patterns, or willingness to ask for help. Physical and occupational therapy personnel may assess balance, strength, mobility, transfers, equipment, and environmental barriers. Pharmacists and prescribing clinicians may identify medications that contribute to sedation, dizziness, hypotension, or other fall risks. Environmental, dietary, social work, and other team members may also contribute important information and interventions.

The goal of fall prevention is not to eliminate movement or restrict independence. Effective prevention focuses on identifying modifiable risks while supporting safe mobility, dignity, function, and participation in daily activities. Interventions should be individualized to the person’s condition, abilities, preferences, and care environment.

This course focuses on common fall risk factors, multidisciplinary assessment and prevention strategies, safe mobility support, environmental safety, medication-related concerns, communication across disciplines, post-fall response, documentation, and role boundaries. Learners will also examine how information from different team members can be combined to identify why a fall occurred and reduce the risk of recurrence.

Learner Objectives

Upon completion of this course, the learner will be able to:

  • Identify common individual, medication-related, functional, cognitive, and environmental factors that may increase fall risk.
  • Apply multidisciplinary fall-prevention strategies that support safe mobility, individualized care, communication, and appropriate role boundaries.
  • Describe appropriate observation, reporting, documentation, and post-fall response when a patient, resident, or client experiences a fall or significant change in fall risk.
