This multidisciplinary course prepares home care professionals to identify fall risks and support safer daily activity for older adults receiving care at home. Learners will review environmental hazards, mobility and transfer concerns, medication and health-related risk factors, assistive devices, footwear, bathroom safety, documentation, communication, and role-appropriate interventions that can reduce fall risk while supporting independence.
Target Audience
Registered Nurses, Licensed Practical/Vocational Nurses, Certified Nursing Assistants, home health aides, personal care aides, physical therapists, occupational therapists, care coordinators, caregivers, and other professionals who provide or support care for older adults in the home.

Overview
Falls in the home can result from a combination of mobility changes, weakness, balance problems, medications, health conditions, unsafe transfers, poor lighting, clutter, footwear, bathroom hazards, and environmental barriers. This course reviews practical fall-prevention strategies for multidisciplinary home care, including observation, environmental safety, assistive devices, transfer support, communication, documentation, and escalation when a patient’s mobility or condition changes. Emphasis is placed on preserving independence while recognizing when additional nursing, therapy, equipment, or caregiver support may be needed.
