Post-Hospital Care at Home: Supporting a Safe Recovery

Target Audience
Registered Nurses, Licensed Practical/Vocational Nurses, Certified Nursing Assistants, home health aides, personal care aides, therapists, care coordinators, caregivers, and other professionals who provide or support care in the home following hospitalization.

Overview
The transition from hospital to home may involve new medications, activity restrictions, wound or drain care, mobility changes, follow-up appointments, and warning signs that require timely attention. This course reviews how multidisciplinary home care staff can support discharge instructions, recognize concerning changes, prevent falls, observe wounds and drains, communicate findings, and respond within appropriate role boundaries. Emphasis is placed on safe recovery, clear documentation, timely escalation, and coordination among HHAs/CNAs, LPNs/LVNs, RNs, therapists, patients, and caregivers.

Learner Objectives
Upon completion of this course, the learner will be able to:

• Identify key components of post-hospital discharge instructions, including medications, activity restrictions, wound or drain directions, follow-up appointments, warning signs, and home safety needs.
• Differentiate role-appropriate responsibilities of HHAs/CNAs, LPNs/LVNs, RNs, therapists, and other home care team members when supporting recovery, wound or drain care, observation, documentation, and escalation.
• Apply appropriate observation, fall-prevention, documentation, communication, and escalation practices when recognizing warning signs or barriers to safe recovery at home.
