Overview

Pressure injury prevention requires coordinated care, but responsibilities differ by role and scope of practice. Direct caregivers may assist with repositioning, skin care, mobility, and observation while promptly reporting skin changes. Licensed nurses may assess skin integrity, evaluate reported concerns, document findings, provide ordered wound care, and coordinate follow-up. Rehabilitation, dietary, medical, and wound care professionals contribute expertise related to mobility, positioning, nutrition, medical conditions, equipment, and treatment planning.

This course focuses on pressure injury risk factors, early recognition, staging concepts, prevention strategies, skin and wound observations, nutrition, moisture, mobility, pressure redistribution, documentation, communication, and escalation. Learners will also differentiate between recognizing and reporting a possible pressure injury and performing assessment, staging, diagnosis, or treatment that requires appropriate clinical authority.

Learner Objectives

Upon completion of this course, the learner will be able to:

  • Identify common pressure injury risk factors, early skin changes, pressure-prone areas, and basic pressure injury staging concepts.
  • Differentiate role-appropriate responsibilities for pressure injury prevention, observation, reporting, assessment, treatment, nutrition, mobility, and care coordination across members of the multidisciplinary team.
  • Apply appropriate prevention, communication, documentation, and escalation actions when skin changes or a suspected pressure injury are identified.
