Overview
Providing hospice care in the home requires a coordinated approach that prioritizes comfort, dignity, safety, communication, and respect for the patient’s established plan of care. Staff may observe changes in symptoms, mobility, skin condition, intake, alertness, breathing, behavior, or caregiver concerns and must respond according to their role, agency procedures, and the hospice plan of care.
This course reviews practical responsibilities when supporting hospice patients at home, including maintaining a safe and comfortable environment, observing and reporting changes in condition, assisting with permitted comfort measures, communicating effectively with patients and families, and collaborating with hospice nurses and other members of the interdisciplinary team. Emphasis is placed on recognizing role boundaries, documenting observations accurately, avoiding independent changes to treatment outside the worker’s scope, and promptly communicating concerns that may require assessment or intervention by the appropriate hospice professional.
