Supporting Plastic Surgery Patients at Home

Target Audience

Home care professionals supporting adults recovering from plastic surgery, including certified nursing assistants, home health aides, registered nurses, licensed practical/vocational nurses, and other direct care personnel. The course emphasizes practical CNA/HHA responsibilities while providing relevant review for licensed nurses involved in postoperative care, supervision, delegation, and care coordination. Learners should apply the content within their own scope of practice and authorized responsibilities.

Overview
Recovery after plastic surgery requires individualized instructions and reliable communication between the patient, surgical team, and home care provider. This course addresses practical support after procedures such as breast surgery, abdominoplasty, liposuction, and facial surgery. Learners will distinguish supportive care and observation from nursing assessment, treatment decisions, and other tasks requiring a licensed professional.

The course will include six instructional sections, with approximately 45 minutes of reading, 10 minutes of scenario-based reflection, and 5 minutes for the post-test:

1. Preparing for care, patient pickup, and safe transportation. Confirming facility discharge, obtaining written instructions and emergency contacts, receiving a handoff, arranging safe transfers and positioning, and confirming the required supervision after arrival home. Aides do not independently determine readiness for discharge.
2. Following postoperative instructions at home. Applying patient-specific directions for activity, positioning, walking, nutrition, fluids, toileting, compression garments, and follow-up visits. Medication assistance must remain within the worker’s authorized role. Conflicting or incomplete instructions require clarification.
3. CNA/HHA responsibilities and limits. Distinguishing observation and reporting from clinical assessment and treatment. This course will teach aides to leave surgical dressings in place and arrange authorized clinical assistance for dressing changes. It will not teach wound treatment, drain removal, or independent changes to the postoperative plan.
4. Observing dressings and assisting with drains. Describing visible drainage without lifting or removing dressings. When specifically directed and permitted, tracing the outer edge of visible drainage on the external dressing and recording the date and time without disturbing the dressing. Marking drainage does not replace reporting concerning bleeding. Emptying a collection bulb and measuring and recording output will be addressed only when permitted, specifically instructed, and supported by training and any required delegation. Permission to empty a drain does not authorize stripping, flushing, repositioning, or removing it.
5. Recognizing and reporting changes. Observing alertness, breathing, mobility, reported pain, intake, and other assigned measures. Documenting objective findings, following patient-specific reporting instructions, and communicating new or worsening symptoms promptly. Collecting a measurement does not authorize an aide to interpret it as proof that the patient is safe.
6. Responding to emergencies. Recognizing situations requiring immediate emergency assistance, activating 911, following dispatcher instructions, and providing first aid or CPR within training. The course will address severe breathing difficulty, unresponsiveness, chest pain, uncontrolled bleeding, and other urgent changes. Learners will practice communicating with responders, notifying the appropriate care contacts, and documenting events. After an emergency, staff should report unresolved risks and changes in the client’s condition.

Throughout the course, the surgical team’s instructions will be considered alongside applicable law, the service setting, the care plan, and the worker’s demonstrated competency. A patient’s request or a discharge instruction alone does not expand a CNA/HHA’s authorized duties.

Upon completion of this course, the learner will be able to:

• Identify essential steps for patient pickup, discharge handoff, safe transportation, and implementation of postoperative instructions at home.
• Differentiate role-appropriate CNA/HHA assistance from tasks requiring licensed clinical care, including dressing observation, authorized drain assistance, documentation, and reporting.
• Recognize postoperative warning signs and describe appropriate responses, including prompt clinical notification, activation of emergency services, and reporting unresolved risks after an emergency.
